Strange values for Part B

The values for the Medicare part B premiums appears range all over the place

(for myself and spouse), when I would expect it to be either constant of at lease just monotonically decrease. I've set no value for growth rate for part b premiums, and only one year (2016) has income > 170,000, the IRS cutoff for joint filing high level income surcharge, so what does these numbers appear to vary all over the place:

The Medicare B premium means test threshold is frozen at $170,000 in nominal dollars until 2019. This means that it's decreasing in terms of constant 2012 dollars.
Additionally, the means test is based on Modified Adjusted Growth Income in the second preceding year. So, your MAGI in 2012 is used to determine your 2014 Medicare B premium.

It can get even more complicated if you are receiving Social Security benefits because of the hold harmless provision that ensures that your SS benefit will not decrease because of increased Medicare B premiums.
