Survivor Benefits
I have a pension that pays a fixed amount for life, not indexed for inflation; with a 50% survivor benefit. I enter the annual amount in the #4 Annual Amount field and I enter in #5 Percentage of annual benefit provided to survivor field as 50%. After clicking the ADD button the box above updates to reveal the correct pension amount, but it lists the survivor benefit as 50% lump sum as opposed to annual benefit.
I noticed that if I enter #5 Percentage of lump sum field as 50% then the ADD button the box above reveals 50% in the Annual Benefit field. I do not know if this is a bug in the program and what affect this has on my profile.

Good catch. This is probably just a display bug, but we'll check it out thoroughly and post an update in a day or so.
